In this role, you will be responsible for screening and conducting intakes for individuals seeking admission to the PROS Program. Conducts assessments to determine individual’s needs and preferences and develops recommendation for the person’s Individual Recovery Plan. Conducts new member orientation and coordinates with all referral sources and collaterals. Maintains appropriate records according to program regulations
HOURS: 40 hours per week
QUALIFICATIONS:
Licensed Master Social Worker. Experience working with individuals in recovery from Mental Illness. Valid and clean NYS Drivers License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

We are looking for a Part-Time Behavioral Health Clinician (LCSW, LPC, LMFT)
The licensed clinician reports to a Supervisor in Outpatient Behavioral Health and is responsible for developing and managing their caseload with appropriate supervision as well as maintaining all documentation requirements. The position will provide clinical services to an assigned caseload. The position requires that clinician will have the ability and flexibility to be responsive to the clinical needs of the agency.
To provide individual, family and possibly group therapy at FCA in outpatient behavioral health department
Responsible for assessments, evaluations, and treatment planning
Notify intake coordinator of potential client openings
Complete all documentation in adherence with specified time frames and other regulatory requirements
Assume responsibility for ongoing professional development
Attend bi-weekly supervision
Schedule clients for medication evaluations as needed
Coordinate care with other FCA programs
Coordinate client care with community agencies
Case management
Ensure that the program’s documentation and services provided meet model regulatory requirements
ESSENTIAL QUALIFICATIONS:
LCSW / LPC / LMFT
Bilingual (Spanish speaking) is preferred
Valid Drivers License
Experience with working with evidence based and integrative psychotherapy treatment models
Must have awareness of the cultural and socioeconomic characteristics of clients served

Job Summary
